Personal Care

Home Health Aids and Personal Care Attendants (PCAs) assist with medication, bathing and more. Other tasks include transfers and positioning, dressing, grooming, exercises, toileting, meal preparation and planning, light housekeeping, assisting with errands and groceries, companionship and general overall safety.

Nursing Care

Trained caregivers with a Medicaid certified agency provides skilled nursing visits in the comforts of one’s home. Examples include coordination of care, client advocacy, general assessment of client’s health status, medication management and set up, post hospitalization and surgical care, diabetic care, injections, wound care, foot, nail and skin care, INR testing for anti-coagulation therapy, disease management, catheter and colostomy maintenance and pain management.

Physical Therapy

Independent Living

Independent Living Skills  (ILS) are taught to help people with disabilities and progressive illnesses learn to live independently in their own homes. Goals such as personal hygiene, interpersonal skills, cooking and nutrition, and communication skills are achieved. Other goals such as financial management, problem solving, transportation and vocational skills, task completion, and medication management are also explored.

Meals and Nutrition

Overall health and well-being begins with proper nutrition. Factors such as loneliness, medications, reduced senses and the ability to shop for groceries and cook can discourage someone from eating properly. Registered Nurse

Registered Nurse/Case Manager

Makes skilled nursing assessments of clients/caregivers in the home care setting that includes physical, psycho-social and environmental components. Develops a care plan in consultation with the primary physician. Coordinates the professional and caregivers involved, to meet the goals of the care plan.

Licensed Practical Nurse

Licensed Pratical Nurse (LPN)

Provides client health care services primarily on a part-time, intermittent basis and under the direction of a registered nurse. Services include preventative, supportive, and rehabilitation activities in accordance with the nursing care plan and physician’s plan of care.

Home Health Aide

Home Health Aide

Provides safe and appropriate assistance with the client’s activities of daily living under the direction of the Case Manager’s Plan of Care and considering client’s age, growth and developmental status, physiological and psycho-social needs. Completes appropriate and accurate documentation of services provided as well as general assessment of how the client is managing at home.

Independent Living Skills Worker

Independent Living Skills Worker (ILS)

An ILS Worker is responsible for aiding the client in implementing the ILS Plan. The ILS Plan, which is developed by the client and his/her team, is a guide that illustrates the steps needed to accomplish the client’s goals. The goals may address important areas in the client’s life such as nutritional management, household management, financial management, and interpersonal relationships. The ILS Worker and client coordinate their efforts to integrate the client to function more independently in his/her life through the ILS Plan.

Social Worker

As a member of the multidisciplinary team, the social worker provides supportive services to help meet patient/family social, emotional, economic and environmental needs in relation to the impact of a terminal illness.

Speech Therapist/Physical Therapist/Occupational Therapist

Speech Therapist/Physical Therapist/Occupational Therapist

Responsible for planning and carrying out speech, physical or occupational therapy assessments and treatment procedures in home with patients and their families.

Personal Companion/Personal Care Assistant (PCA)/Companion

Personal Companion/Personal Care Assistant (PCA)/Companion

Provides safe and appropriate assistance with the client’s activities of daily living under the direction of the Case Manager’s Plan of Care and considering client’s age, growth and developmental status, physiological and psycho-social needs. Works with the family and client to maintain a safe environment. Works with client and family to promote the client’s independence based on client age, growth and developmental status and physiological and psycho-social needs.
